All memberships are initiated by a one time enrollment fee of $150.00 per person. This secures the patient spot in the practice, allows for obtaining of patient specific supplies, and covers review of medical records. 

Families sharing one household will have their enrollment fees capped at $300.00 and monthly membership capped at $400/month. To qualify for family discount; at least one parent must enroll, all children must be under the age of 18, and enrolled children must reside at the same address as the enrolled parent. 

Individuals who elect to leave the practice will only have coverage for 30 days after discontinuation of membership. Non-payment of membership fees constitutes discontinuation of membership. 

If an individual leaves the practice, and elects to return, the enrollment fee will be required prior to re-starting membership. There is no fee for membership cancellation, but if the individual would like to return to the practice in the future the enrollment fee and unpaid months membership will be required at time of re-enrollment.
